I guess the question is:
1) What version are you on?
2) Which module / version were you using?
3) Is any of this customized?

1) Latest version (in fact that might be the problem as it was working before)
2) Admin add NPC_Warrior - trying to add a warrior to table
3) Not adding a table via Admin
