Number of names not used: More than 14 billion. Number of names that have been used: probably around 467. There is literally an infinite number of names not in use.

Just choose something that fits your world. It shouldn't be hard to think of something that hasn't been used. If for some reason you really can't think of anything, just give the game the same name as your fantasy world, and add "Online" after it. Tons of games have done it, and tons more will in the future. If the name of your world is the same as another fantasy world, that would be a problem, as that would be IP infringement even if the game world name is not in the title.

If your still having problems coming up with names, you could always use a name generator. I know it sounds tacky, but a lot of people use it...just throwing that out there as well!

The scary thing about name generators is that they usually aren't 100% random. I have gotten the same name within 3 or 4 results a couple of times, which means someone else may be using the same name as you in their copyrighted work. While I don't fully understand the legality of who owns the name that comes from a generator, it could probably be argued 20 different ways successfully in court.

What I also do, more so over is look at my DVD collection and take a few letters randomly from different titles and create a name. I wrote a screen play and the spells they used, were done that way. For example:

Movie Titles:
Dogma
Waiting
Transformers
Contact
Independence Day

Idea:
Dotraind (Dogma, Transformers and I.D.)
Trapenma (Transformers, I.D. and Dogma)
Cowaima (Contact, Waiting and Dogma)

Just some examples on how I have used that approach...

I remember in a William Gibson novel he touched on the idea of basically every real name had been taken and you had to have a program generate a stupid sounding name for you, where the program would check all copyrighted names. It's absurd - I think the law is still a lumbering medieval dinosaur - all of this supposed intellectual property - these are words, a shared thing. It's like saying you own the human DNA cause you mapped it.

This fear that's getting in the way of creativity - it's just the teritorialist attutude of another era clawing its way into today (and making modern day lords out of some, and peasants out of others - though no one wants to admit to being a peasant, so they keep supporting the whole thing)

well the real question he asked
What is the first MMORPG Game name what comes into your head WHAT HASNT BEEN USED!!!!!!! and the word is "FANTASY"
which really makes no sense but I am assuming he wants a name for a mmo game that is fantasy.

First fantasy game name that comes to my mind...."Throbbing Swords" :) ...oh wait you mean that type of fantasy,

I don't believe in other people naming your game. If the passion is there a name will be too.

Well, you have to keep in mind the difference between copyrights and trademarks. Trademarks absolutely make sense. If I made an MMO and called it "World of Warcraft", the only reason I would do that in today's world would be to attempt to defraud players and make money off of someone else's brand, possibly causing damage to that brand ("Ohh, yeah I played 'World of Warcraft', and they ripped me off and kicked me off the server. Don't play it!"). Trademarks/servicemarks simply prevent people from passing off knock-off goods or services, or goods or services that may be misleading. I could likely get away with calling a new pair of shoes "Warcraft", provided it is written completely differently, and has no fantasy theme on it whatsoever. Not that I see the reason why anyone would, you probably could without being sued because it would be obvious that I would not be attempting to make a misleading product. You may still end up in court over it, but typically things like that get thrown out, where it is basically impossible for anyone to draw a single parallel between the two products beyond the name.

(Edit: Of course, because of the "size and scope" of the World of Warcraft brand, almost any usage could get you sued due to the massive branding campaign. At that point, it is believable that there could be World of Warcraft shoes, lighbulbs, deli meats, etc., and once a brand grows to that size, there really is no safe alternate usage of their trademarks. I was merely using it as an example, but a more realistic example would be something like "O'Reiley". As most of you may know, there is both a car parts store named "O'Reiley" as well as a technical book publishing company named "O'Reiley", and they are both trademarked. For a brand of that size and scope, that works. What they are doing is merely preventing another car parts store or another book publisher from taking the name "O'Reiley", which could cause customers to be mislead.)

Copyright laws, on the other hand, do get a bit absurd. Because of Harry Potter, I can never have a character named Voldemort in anything I create? While it could be argued different ways, in theory, no I could not because that is a specific, named thing in a copyrighted work. What if I had an elderly character in an off-off-Broadway dialog-driven play named "Percy Wallace Voldemort Humperdink". Obviously, I am not stealing any material beyond the name, but in copyright law, that could still be a problem.
